What is guarantee of payment?

When an employee seeks care at an in-network facility within their covered benefits, the insurer (or its authorized administrator) may guarantee payment for part or all of the cost, per the policy contract. Employees may still need to pay for:

  • Co-payment amounts
  • Costs exceeding the benefit limit
  • Excluded items
  • Costs outside the scope of the guarantee

Guarantee of payment does not mean "nothing to pay."

What is reimbursement?

Reimbursement typically applies when an employee has already paid out of pocket (for example, when visiting an out-of-network facility or when a cashless arrangement wasn't available), then submits a claim to be repaid according to the contract's rate and conditions. Incomplete documentation is a common reason claims take longer to process.
